​​IMPORTANT NOTICES REGARDING PHYSICAL EXAMS:

• All new students and students entering grades K, 1, 3, 5, 7, 9 & 11 are required to provide a health examination form.   
• Health exams must be dated within 12 months of the first day of school. 
• If your child plans to sign up for Modified Sports (7th & 8th grade only), the physical exam must be dated within 12 months of the start date of the sport.
