Effective July 1, 2018
Health Examination changes:
Section 136.3 regarding Health Examinations and Screenings will change. The new regulations for health examinations for attendance will be required for new entrants and in grades Pre-K or K, 1, 3, 5, 7, 9, and 11.
Hearing and Vision screenings will be done in grades 5 & 7.
Vision screenings will now include near vision, as well as distance acuity.
Scoliosis screenings will be done for 5th & 7th grade girls only. Boys will be screened at CHHS in 9th grade.

- The only form that may be used to document health examinations for students attending public school is the approved form titled; NYSED (New York State Education Department) Student Health Examination Form (https://www.p12.nysed.gov/sss/documents/health-exam-form.pdf)
- The new NYS School Health Examination Form is not required for physicals done prior to 7/1/18.
- To assist with the transition, during the 2018-2019 school year schools may accept any health examination form provided.
- In 2019-2020 ONLY the approved form will be accepted.
