The end of the 2022-23 school year is quickly approaching. If your child has medication in the PVC Health Office, please remember to:
• Pick up your child’s medication(s) by 10am on 6.23.23. Please note that students are not permitted to pick up or transport medications from/to school. • No medications can be held over until September. • Any medications left in the Health Office after 10am on 6.23.23 will be discarded.
• ANY MEDICATIONS (Rx or OTC) for the 2023-24 School Year require new healthcare provider orders using the CHUFSD Medication Administration form found at http://pvcschoolnurse.weebly.com/forms.html. In addition, please ensure your child’s provider identifies her/him as dependent, supervised or independent. No medication will be accepted or administered without this form completed by your child’s provider and signed by you.
• For any medication stored in the Health Office, best practice is for you to deliver medications to the Health Office:
1. in their original container 2. with the medication’s name 3. with your child’s name 4. dosage 5. time of administration 6. # of doses 7. name of the prescribing provider and contact information
